As we approach the end of 40 years of publishing the International Journal of Medical Informatics, we are gratified with both the growth of our field and the growth of our journal. As hospitals, health systems, and governments have focused on the problems of costs and quality in healthcare, many have turned to computers to seek solutions. The scientific literature helps build the foundations for innovations and is a repository for the wealth of accumulated experience of what has worked and what has not. About a decade ago, we made an editorial decision to focus on applications that had strong evaluation components.
